AMT Physiotherapy Terms, Privacy & Booking

1. Privacy & Data Protection Policy (UK GDPR)

AMT Physiotherapy, in Consett, is committed to ensuring your personal data is handled lawfully, fairly, and securely in accordance with UK GDPR. All patient information is treated with the strictest confidentiality.

Data Collection & Use

We collect your name, contact details, medical history, and clinical treatment records solely to provide safe, effective physiotherapy treatment, manage appointment communications, and liaison with other healthcare professionals (only with your explicit consent).

Clinical Record Keeping & Storage

Clinical notes are completed after each session and include assessment findings, treatment, and ongoing rehabilitation plans. To ensure accurate and secure records, we utilise Cliniko, an secure online booking and recording system recommended by the Chartered Society of Physiotherapists.

  • Digital systems are secure and password-protected.

  • Any paper records are stored securely in locked cabinets.

  • In line with legal requirements, adult records are retained securely for 8 years, after which they are permanently and safely disposed of.

Your Rights

You retain the right to access your data, request corrections, or withdraw your consent at any time. Any potential data breaches will be recorded and reported strictly in line with Information Commissioner’s Office (ICO) guidance.

2. Consent Policy

AMT Physiotherapy is committed to obtaining valid, informed consent for all services provided, ensuring you remain a fully informed partner in your recovery.

The Consent Process

Prior to any assessment or treatment, you will receive a clear explanation regarding:

  • Your clinical condition and diagnosis.

  • Proposed treatment options and rehabilitation paths.

  • Associated risks, benefits, and potential alternatives (including the implications of no treatment).

Administration

  • Routine Care: Consent is typically verbal for routine assessment and treatment.

  • Specialised Care: Written consent will be required where appropriate, such as for group classes or specific sensitive treatment techniques.

  • Autonomy: You may withdraw your consent at any time. Capacity to consent will be formally assessed where required by clinical guidelines, and all consent decisions are clearly documented within your clinical notes.

3. Appointment & Cancellation Policy

Booking Requests

Appointments must be booked in advance. This can be completed quickly through our online booking system, or by contacting the clinic directly via email or telephone.

Terms, Privacy & Booking

We value your time and request that you value ours. A minimum of 24 hours’ notice is required for any appointment cancellations or rescheduling requests. Late cancellations or missed appointments without prior notice may be charged in full.

4. Payment PolicyFees & Settlement

All clinic fees are clearly displayed and agreed upon prior to your treatment. Payment is due at the time of your appointment.

Accepted Payment Methods

We accept the following payment methods for your convenience:

  • Bank transfer

  • Cash

  • Debit or Credit card payments

Please note that refunds are not issued for completed clinical treatments.

5. Stay Steady Programme Policy (Group Classes)

To ensure the safe and effective delivery of our group rehabilitation sessions, the following procedures apply to the Stay Steady Programme:

  • Initial Assessment: A 1-to-1 clinical assessment is required before joining any group class to ensure the programme is safe for you.

  • Tailored Care: All exercises are specifically tailored to your individual ability and mobility level.

  • Supervision: Group sizes are strictly limited to ensure close, safe clinical supervision.

  • Safety & Emergencies: Detailed safety instructions are provided at each session. Full first aid procedures are in place, and emergency contacts are securely recorded for every participant.
